ABSTRACT

Introduction: The cecum is the first part of the large bowel. Cecectomy is a sufficient treatment for some patients, avoiding overtreatment by ileocolic resection. Purpose:The goal of this study was to review a surgeon's experience with laparoscopic cecectomy and provide a technical video demonstration of this uncommon operation. Methods: A retrospective chart review was conducted of all consecutive patients treated with laparoscopic cecectomy over a 16-year period. All operations were performed using a 3-trocar technique. The cecum was transected with 1 to 2 firings of a 60 millimeters linear stapler, preserving the ileocecal valve and ascending colon. Results: 19 patients were identified including 12 females (63.2%). Median age was 42 years (range 16-84). Indication for surgery included appendiceal pathology in 12 patients (63.2%) and cecal abnormality in 7 (38.9%). There was no conversion to open surgery. Median intraoperative blood loss was 25 ml (range 0-150 ml) and no patient received a blood transfusion. No intraoperative or postoperative complication was noted. The median length stay was 1 day (range 0-6). Readmission rate was 0%. Final appendiceal histopathology revealed acute/chronic appendicitis in 5 patients, mucinous cystadenoma in 4 patients. Cecal histopathology revealed adenoma in 4 patients. Median follow-up was 16 months (range 4-53). Conclusions: Laparoscopic cecectomy is a sufficient treatment for some patients with benign conditions of the appendix and cecum. It carries minimal morbidity. It should be considered as an alternative to segmental bowel resection in a select group of patients. (AU)

ABSTRACT

Gastrointestinal submucosal lesions represent a diagnostic challenge, including benign or malignant lesions, so they are identified more accurately by histopathological study accompanied by immunohistochemistry. This is a case of a 21-year-old man with a bleeding submucosal lesion in the cecum. The patient underwent a right colectomy. Microscopic finding was compatible with Vanek's tumor.


Lesiones submucosas gastrointestinales representan un reto diagnóstico, incluyen lesiones benignas o malignas, que se identifican con mayor exactitud con estudios histopatológicos, incluyendo inmunohistoquímica. Se reporta el caso de un paciente varón de 21 años con lesión submucosa hemorrágica localizada en el ciego, por lo que se realiza colectomía. Hallazgos microscópicos son compatibles con Tumor de Vanek.

ABSTRACT

La angiostrongiliasis abdominal es una enfermedad parasitaria causada por el género Angiostrongylus costaricensis, un nematodo que infecta a los humanos a través de las secreciones de caracoles o babosas, sus huéspedes definitivos. Costa Rica es el país que se considera más endémico, se ha evidenciado en distintas revisiones que la mayoría de los casos se presentan en niños y personas del sexo masculino. Presentación del caso. Se trata de una mujer de 74 años, que consultó por dolor abdominal agudo, de cuatro días de evolución, acompañado de náuseas, vómitos e hiporexia. Los exámenes de laboratorio reportaron leucocitosis leve y examen general de orina negativo. La ultrasonografía abdominal reportó una masa sólida, heterogénea, mal circunscrita en flanco derecho, esto llevó a la sospecha de una apendicitis aguda. Intervención terapéutica. Se realizó una laparotomía exploradora, con la extirpación de una tumoración de colon perforado que abarcaba desde el ciego hasta el tercio proximal de colon ascendente, además, se realizó una anastomosis de íleo transversa, sin mayor complicación. Evolución clínica. Se manejó con antibióticos y analgésicos, fue dada de alta a los nueve días posteriores al procedimiento, sin complicaciones menores. La biopsia reportó huevos de parásitos consistentes con Angiostrongylus sp


bdominal angiostrongyliasis is a parasitic disease caused by the genus Angiostrongylus costaricensis, a nematode that infects humans through the secretions of snails or slugs, its definitive hosts. Costa Rica is considered the most endemic country, and it has been shown in different reviews that most cases occur in children and males. Case presentation. The patient was a 74-year-old woman who consulted for acute abdominal pain, with four days of evolution, accompanied by nausea, vomiting, and hyporexia. Laboratory tests reported mild leukocytosis and a negative general urine test. Abdominal ultrasonography reported a solid heterogeneous mass, poorly circumscribed in the right lateral abdomen, which led to the suspicion of acute appendicitis. Treatment. An exploratory laparotomy was performed, with the removal of a perforated colon tumor that spanned from the cecum to the proximal third of the ascending colon, in addition, a transverse ileum anastomosis was performed, without further complication. Outcome. She was managed with antibiotics and analgesics and was discharged nine days after the procedure, without complications. The biopsy showed parasite eggs consistent with Angiostrongylus sp

ABSTRACT

Introducción. La oclusión intestinal, completa o incompleta, es uno de los cuadros de abdomen agudo más frecuentes. Constituye entre 20 % y 35 % de los ingresos urgentes en las áreas quirúrgicas hospitalarias. Caso clínico. Se presenta el caso de un paciente con antecedente de carcinoma de próstata, que consultó con un cuadro de obstrucción intestinal y abdomen agudo. Se le diagnosticó vólvulo de ciego, mal rotación intestinal y situs ambiguous. El tratamiento quirúrgico del paciente fue exitoso. Conclusión. El conocimiento de estas condiciones patológicas es imprescindible para poder brindarle un correcto tratamiento quirúrgico y disminuir la mortalidad que pueden acarrear


Introduction. Complete or incomplete intestinal obstruction is one of the most frequent acute abdomen conditions. It constitutes between 20% and 35% of the urgent admissions of hospital surgical areas. Clinical case. We present a case of a patient with a history of prostate carcinoma, who began with clinical symptoms of intestinal obstruction and acute abdomen. He was diagnosed with volvulus of the cecum, intestinal malrotation, and situs ambiguous. Surgical treatment of the patient was successful. Conclusion. Knowledge of these pathological conditions is essential to be able to provide correct surgical treatment and reduce the mortality that these can lead to

ABSTRACT

Antibiotic associated diarrhoea (AAD) is caused mostly by disruption of the physiological gut microflora. One potential strategy to prevent this is the concurrent use of probiotic bacteria or yeast. Tribal populations of West Garo Hill region of Meghalaya use locally available unique microflora to prepare fermented food products whichserve as a potential source of novel probiotic organisms and bioactive compounds. In this context, here, we have evaluated the potentiality of laboratory made functional fermented rice beverage in mitigating AAD on animal models viz. Wistar rats. The animal models were administered with fermented rice beverage (test group A1), combination of indigenous Lactobacillus isolates (test group A2), a yeast isolate (test group A3), loperamide (test group STD), normal control (test group NC) and disease control (test group DC). Furthermore, various diarrhea assessment parameters were checked from each group followed by analysis of fecal microbiome, haematological parameters, histopathology of colon, liver and cecal short chain fatty acids (SCFAs) determination. NC and A1 was least affected by AAD induction with a faecal consistency score of 1 on the final day of the study. After day 10, a significant reduction (P <0.05) in the faecal water content was observed in A1, A2 and STD till day 15. Compared to the NC, a slight decrease in body weight was found in the rest of five test groups at day 5, 10 and 15. Except NC, the remaining five test groups showed a significant decrease of lactobacilli and yeast counts in faecal microbiota at 5th day. An increase in the enterococci and coliform counts indicated severe diarrheal condition but A1 reported with significant increase (P <0.05) in the population of Lactobacillus at day 15. An increase in red blood corpuscles, haemoglobin, packed cell volume, mean cell haemoglobin, mean corpuscular volume, mean corpuscular hemoglobin concentration was observed. During the histopathology analysis of colon and liver, normal layers of mucosa, submucosa, muscularis and serous with absence of any abnormal changes or lesions was seen in A1. The cecal concentrations of lactate, acetate and propionate were significantly higher in A1 as compared to the other test groups. Therefore, fermented rice beverage possesses potential to be used in inhibition of antibiotic associated diarrhea with further clinical investigations.

Introducción. La esclerosis sistémica es una enfermedad del colágeno de etiología autoinmune, con manifestaciones gastrointestinales hasta en un 90 % de los pacientes. Aunque es infrecuente, se han descrito algunos casos de vólvulos colónicos, pero es extremadamente rara la presentación de vólvulo cecal y del colon sigmoide en un mismo paciente. Caso clínico.Paciente femenina de 65 años, con antecedente de esclerosis sistémica, quien consultó a urgencias por dolor y distensión abdominal, asociados a emesis, con ausencia de flatos y deposiciones. Por imágenes sugestivas de obstrucción intestinal con zona de transición en el colon sigmoide se indicó laparotomía exploratoria, con hallazgo de vólvulo del sigmoide. Posteriormente reingresó por sintomatología similar, con imágenes sugestivas de vólvulo cecal. Se realizó hemicolectomía derecha con ileostomía y posterior cierre de ileostomía en misma hospitalización. Discusión. El vólvulo del colon sigmoide y ciego en un mismo paciente es una condición muy infrecuente. Existen pocos casos reportados en la literatura. Conclusión. La resección del segmento colónico afectado es el estándar de manejo quirúrgico.


Introduction. Systemic sclerosis is a collagen disease of autoimmune etiology, with gastrointestinal manifestations in up to 90% of patients. Although infrequent, some cases of colonic volvulus have been described, but the presentation of cecal and sigmoid colon volvulus in the same patient is extremely rarely. Clinical case. A 65-year-old female patient, with a history of systemic sclerosis, who consulted the emergency room due to abdominal pain and distension, associated with emesis and absence of flatus and stools. Due to images suggestive of intestinal obstruction with a transition zone in the sigmoid colon, an exploratory laparotomy was indicated, with the finding of a sigmoid volvulus. She was later readmitted for similar symptoms, with images suggestive of cecal volvulus. Right hemicolectomy was performed with ileostomy and subsequent closure of the ileostomy in the same hospitalization. Discussion. Volvulus of the sigmoid and cecum in the same patient is a very rare condition. There are few cases reported in the literature. Conclusion. Resection of the affected colonic segment is the standard of care management.

RESUMEN Las úlceras solitarias de ciego (USC) son infrecuentes y no se registran aún en la literatura de la Ar gentina. Existen informes de casos y pequeñas series en otros países. Son de etiología desconocida y diagnóstico diferencial amplio. En este caso se resolvió mediante una hemicolectomía derecha supra selectiva por laparotomía de emergencia.


ABSTRACT Solitary cecal ulcers (SCUs) are rare and no cases have been published in Argentina. Few cases or series have been reported in other countries. The etiology is unknown and many other diseases must be ru led out before making the diagnosis. This case was solved with selective right hemicolectomy through laparotomy in an emergency basis.

RESUMEN El vólvulo de ciego involucra al colon derecho proximal, el ciego y el íleon terminal. Se necesitan dos requisitos, una fijación incompleta del ciego y la limitación del movimiento intestinal en un punto fijo que sirve como apoyo y eje para la rotación. Presentamos el caso de una mujer de 52 años que presentó una obstrucción intestinal aguda. Se hizo el diagnóstico preoperatorio con una tomografía y se realizó una colectomía derecha asistida por laparoscopía con anastomosis primaria. En estos pacientes se recomienda la colectomía derecha no oncológica ya que elimina por completo la posibilidad de recurrencia. No se recomienda la colonoscopía, la cecopexia, ni la cecostomía por su alto riesgo de recurrencia y morbimortalidad asociadas.


ABSTRACT Cecal volvulus involves the proximal right colon, the cecum and the terminal ileum. Two requirements are needed, an incomplete fixation of the cecum and the restriction of the bowel movement at a fixed point which serves as a fulcrum for rotation. We present a case of a 52-year-old woman who was admitted with the diagnosis of an acute intestinal obstruction. The preoperative diagnosis was made with a tomography and she underwent a laparoscopic assisted right colectomy with primary anastomosis. In these patients, the right colon nononcologic surgical resection is recommended because eliminates the possibility of volvulus recurrence. Colonoscopy, cecopexy, or cecostomy should be avoided because its associated significant recurrence, morbidity and mortality.

BACKGROUND: Lignocellulose is considered a renewable organic material, but the industrial production of biofuel from lignocellulose is challenging because of the lack of highly active hydrolytic enzymes. The guts of herbivores contain many symbiotic microorganisms that have evolved to hydrolyze plant lignocellulose. Chinese bamboo rats mainly consume high-fiber foods, indicating that some members of the intestinal tract microbiota digest lignocellulose, providing these rats with the energy required for growth. RESULTS: Here, we used metagenomics to analyze the diversity and functions of the gut microbiota in Chinese bamboo rats. We identified abundant populations of lignocellulose-degrading bacteria, whose main functions involved carbohydrate, amino acid, and nucleic acid metabolism. We also found 587 carbohydrate-active enzyme genes belonging to different families, including 7 carbohydrate esterase families and 21 glycoside hydrolase families. The glycoside hydrolase 3, glycoside hydrolase 1, glycoside hydrolase 43, carbohydrate esterase 4, carbohydrate esterase 1, and carbohydrate esterase 3 families demonstrated outstanding performance. CONCLUSIONS: The microbes and enzymes identified in our study expand the existing arsenal of proficient degraders and enzymes for lignocellulosic biofuel production. This study also describes a powerful approach for targeting gut microbes and enzymes in numerous industries.

Background: Hernia of the umbilical cord is a rare clinical entity which presents with hernia of the small bowel into the proximal part of the umbilical cord. This is sporadically associated with other congenital malformations. This is usually poorly identified and mistakenly termed as 'omphalocele minor. Inadvertent clamping of the cord in these cases leads to iatrogenic bowel wall injury. The aim of this study is to present a spectrum of cases presenting with umbilical cord hernia in a tertiary care Govt medical college, along with demography, intra operative findings, associated malformations and postoperative outcome and 1 year follow up.Methods: This is an ambispective study on neonates who attended the Outpatient Department (OPD) or Emergency department. The babies were evaluated by age, birth weight, gender, any gross clinical malformation. They were also evaluated intraoperatively for any associated anomalies. Post-surgery the babies were followed up until 2 years of age in pediatric surgery OPD of the same institute and outcome was recorded.Results: Out of 90 babies, 88 babies (97.77%) had body weight more than 2.5 kg, and rest 2(2.22%) had bodyweight of 1.5kg and 1.8 kg. Regarding content of contents of umbilical cord hernia, out of 87 patients, 43 patients (49.42%) had ileal loop as, 32 patients (36.78%) had ileum with Meckel's diverticulum, 6 patients (6.39%) had cecum with appendix.Conclusions: Most of this study cases had ileal loops as content of the hernial sacs, and 36.78% cases had Meckel's diverticulum associated, which is a remnant of Vitello intestinal duct. Prompt identification of the condition and early intervention and adequate post-natal care are affective to prevent long term morbidity.

Introducción. El vólvulo cecal en el embarazo es una patología infrecuente a nivel mundial, con un espectro clínico variado y limitación en estudios diagnósticos, lo que puede resultar en un desenlace ominoso. Caso clínico. Se presenta el caso de una gestante en segundo trimestre del embarazo quien presenta abdomen agudo secundario a vólvulo cecal y sufrimiento fetal. La paciente fue llevada a cesárea de urgencia, donde además se practicó cecectomía y anastomosis íleo transversa, con mortalidad fetal. Discusión. El vólvulo cecal forma parte de los diagnósticos diferenciales en caso de abdomen agudo durante el embarazo. Tiene un pronóstico reservado, en donde pueden presentarse desenlaces maternos y perinatales adversos, con mortalidad materno fetal


Introduction. Cecal volvulus in pregnancy is an uncommon pathology worldwide, with a varied clinical spectrum and limitation in diagnostic studies, which can result in an ominous outcome.Clinical case. We present the case of a pregnant woman in the second trimester of pregnancy who presents with an acute abdomen secondary to caecal volvulus and fetal distress. The patient was taken to emergency caesarean section, where a cecectomy and transverse ileus anastomosis were also performed, with fetal mortality.Conclusion. Cecal volvulus is part of the differential diagnoses in acute abdomen during pregnancy. It has a reserved prognosis, where adverse maternal and perinatal outcomes can occur, including maternal-fetal mortality

ABSTRACT

Sepsis is an infection-induced systemic inflammatory syndrome. The immune response in sepsis is characterized by the activation of both proinflammatory and anti-inflammatory pathways. When sepsis occurs, the expression and activity of many inflammatory cytokines are markedly affected. Xenobiotic receptors are chemical-sensing transcription factors that play essential roles in the transcriptional regulation of drug-metabolizing enzymes (DMEs). Xenobiotic receptors mediate the functional crosstalk between sepsis and drug metabolism because the inflammatory cytokines released during sepsis can affect the expression and activity of xenobiotic receptors and thus impact the expression and activity of DMEs. Xenobiotic receptors in turn may affect the clinical outcomes of sepsis. This review focuses on the sepsis-induced inflammatory response and xenobiotic receptors such as pregnane X receptor (PXR), aryl hydrocarbon receptor (AHR), glucocorticoid receptor (GR), and constitutive androstane receptor (CAR), DMEs such as CYP1A, CYP2B6, CYP2C9, and CYP3A4, and drug transporters such as p-glycoprotein (P-gp), and multidrug resistance-associated protein (MRPs) that are affected by sepsis. Understanding the xenobiotic receptor-mediated effect of sepsis on drug metabolism will help to improve the safe use of drugs in sepsis patients and the development of new xenobiotic receptor-based therapeutic strategies for sepsis.

RESUMEN Las patologías isquémicas a nivel de colon tienen una clínica poco característica, por ello, son de difícil diagnóstico. Se reporta a un paciente, con antecedente de hipertensión arterial, que presenta dolor abdominal de 12 horas de evolución, localizado en fosa iliaca derecha sin irradiación, tipo cólico, de moderada intensidad, sin síntomas acompañantes. Al examen físico presenta abdomen depresible, doloroso a palpación profunda, signos de Mc Burney, Rovsing y Blumberg positivos. El hemograma reporta leucocitosis 12.900/mm3 con neutrofilia 82,9%. Se diagnostica apendicitis aguda y se planea apendicetomía, pero el cirujano constata apéndice cecal grado II, base apendicular en buen estado, asas intestinales distendidas e isquemia en la cara anterior del ciego, siendo este hallazgo importante para cambiar la conducta de la cirugía a una ileostomía de urgencia. Se considera que es de importancia conocer los patrones evolutivos de la isquemia de colon y los factores que predicen su curso desfavorable y mortalidad.


ABSTRACT The colon ischemic pathologies have scare clinical features and are difficult to diagnose. We report a case of a with history of arterial hypertension, who presents with abdominal pain of 12 hours in the right iliac fossa without radiation, colic type, of moderate intensity, without other accompanying symptoms. On physical examination the abdomen was soft depressible, painful to deep palpation, signs of Mc Burney, Rovsing and Blumberg were positives. The hemogram reported leukocytosis 12900 with neutrophilia 82.9%. It is diagnosed as acute appendicitis and was planned an appendectomy. The surgeon reported an cecal appendix grade II, good appendicular base, distended bowel loops and ischemia on the anterior face of the cecum, being the cast finding an important one to change the planned surgery to an ileostomy urgent. We consider an important to fact know the evolution patterns of colon ischemia and factors that predict its unfavorable course and mortality.

ABSTRACT

Resumen Diverticulitis of the cecum is a rare cause of acute abdomen. We present the case of a 20-year-old female who was admitted with signs and symptoms consistent with acute appendicitis. However, cecal diverticulitis is evidenced intra-operatively and a right colectomy is performed followed by an ileocolic anastomosis. The patient recovered well and was discharged home on post-operative day five. This is the first known case in a patient of this age. Although rare, it is important to include cecal diverticulitis as a differential diagnosis in such scenarios as well as understand the many surgical techniques available for its management.


Abstract La diverticulitis del ciego es una causa rara de abdomen agudo. En esta ocasión se presenta el caso de una paciente de 20 años de edad quien ingresa con signos y síntomas de apendicitis aguda; durante el procedimiento quirúrgico se evidencia diverticulitis en ciego y se realiza hemicolectomía derecha más íleo transversoanastomosis, evolucionando adecuadamente y con egreso hospitalario satisfactorio. Este es el primer caso conocido en un paciente de esa edad. A pesar de ser una patología infrecuente, es importante tenerla en cuenta como diagnóstico diferencial de apendicitis aguda, y además el cirujano debe estar preparado para aplicar los diversos tratamientos que existen para la resolución de los cuadros.

Antecedentes: la diverticulitis cecal es una patología poco común en los países occidentales. Clínicamente es indistinguible de una apendicitis aguda. Objetivos: exhibir los resultados de acuerdo con diferentes abordajes terapéuticos. Material y métodos: se presentan cinco casos de diverticulitis cecal tratados en nuestra institución entre enero de 2013 y diciembre de 2015. Revisión retrospectiva de historias clínicas e imágenes. Revisión de la literatura. Resultados: fueron incluidos cinco pacientes. En cuatro hubo resolución quirúrgica y uno tuvo buena evolución con tratamiento médico. Conclusiones: si bien es poco frecuente, la diverticulitis cecal debe considerarse dentro de los diagnósticos diferenciales frente a un cuadro de dolor abdominal localizado en fosa ilíaca derecha acompañado de estudios por imágenes no categóricos de apendicitis aguda.

Review of the literature. Results: five patients were included. Four cases required surgical treatment while one patient resolved with medical treatment. Conclusions: Although it is rare, cecal diverticulitis must be considered within the differential diagnoses in the face of abdominal pain located in the right iliac fossa and non-categorical imaging of acute appendicitis.

ABSTRACT

BACKGROUND: Gut microbiota contributes to intestinal and immune homeostasis through host-microbiota interactions. Distribution of the gut microbiota differs according to the location in the gastrointestinal tract. Although the microbiota properties change with age, evidence for the regional difference of gut microbiota has been restricted to the young. The aim of this study is to compare the gut microbiota between terminal ileum and cecum of old rats. METHODS: We analyzed gut microbiome of luminal contents from ileum and cecum of 74-week-old and 2-year-old rats (corresponding to 60-year and 80-year-old of human age) by metagenome sequencing of 16S rRNA. RESULTS: Inter-individual variation (beta diversity) of microbiota was higher in ileum than in cecum. Conversely, alpha diversity of microbiota composition was higher in cecum than in ileum. Lactobacillaceae were more abundant in ileum compared to cecum while Ruminococcaceae and Lachnospiraceae were more enriched in cecum. The proportions of Deltaproteobacteria were increased in cecal microbiota of 2-year-old rats compared to 74-week-old rats. CONCLUSIONS: Major regional distinctions of microbiota between ileum and cecum of old rats appear consistent with those of young rats. Age-related alterations of gut microbiota in old rats seem to occur in minor compositions.

ABSTRACT

BACKGROUND/AIMS: Because of the national screening program for colorectal carcinoma in The Netherlands, the number of colonoscopies has increased. In case of incomplete colonoscopy, computed tomography colonography (CTC) and double-balloon colonoscopy (DBc) are alternative options. This study evaluated cecal intubation rate and pathology detection rate in the previously unexplored part of the colon, complication rate of DBc, and CTC results after incomplete colonoscopy. METHODS: Retrospective observational study in a tertiary referral hospital regarding DBc and CTC reports from cases with incomplete colonoscopy. RESULTS: Sixty-three DBcs were performed after incomplete colonoscopy. Cecal intubation rate was 95%. Detection rate was 58% (5% carcinoma and 3% high-grade dysplastic adenoma). CTC preceded 54% of DBcs and 62% of CTC findings were confirmed. In 16%, a biopsy was taken, and in 60%, an intervention (mostly polypectomy) was performed. One major complication (1.5%) occurred, i.e., arterial bleeding due to polypectomy necessitating right hemicolectomy. CTC (n=213) showed a possible lesion in 35%, and could be confirmed by follow-up endoscopy or surgery in 65%. CONCLUSIONS: DBc is effective and safe for completion of colon inspection in incomplete colonoscopy. In patients with a high likelihood of pathology, DBc is preferred over CTC.

ABSTRACT

From December 2011 to August 2017,56 patients with acute abdomen surgery were treated in Affiliated Zhongshan Hospital of Dalian University.The anatomical structure of ileocecal Treves folds was observed during operation,and 15 samples of Treves folds were taken for histological examination.The mean length of the fold was (5.56±0.18)cm for males and (5.22±0.05)cm for female,the width was (1.82± 0.04) cm for males and (1.78 ± 0.06) cm for females;the thickness was (0.18 ± 0.02) cm.HE and verhoeff staining showed that Treves fold was mainly composed of elastic fiber,collagen fiber and smooth muscle with abundant blood supply.Thirty patients were repaired with their own Treves folds,including the fixation with overlay suture to the stump of appendectomy in 26 cases,repairing for perforation of colonic diverticulum in 3 case,and repairing for perforation of colonic diverticulum in 1 case.There were no complications such as fecal fistula,intestinal obstruction and intussusception after operation.The Treves fold has a constant location,which can be used as a clear sign of ileal terminal.It has abundant blood supply,and is suitable for the repair and reconstruction in ileocecal operation.

ABSTRACT

A pericecal hernia is a rare form of an internal hernia, which has been described in several case reports. We present a case of a 32-year-old woman who complained of vague abdominal pain a day prior to admission. Abdominal computed tomography revealed the presence of a pericecal hernia without bowel ischemia. The patient underwent manual hernia reduction and was discharged without complications. We describe this case in detail and provide a review of the pertinent literature.

ABSTRACT

PURPOSE: Local recurrence is a common failure pattern in adenocarcinoma of the cecum. This study aimed to investigate the potential role of adjuvant radiation therapy on oncologic outcomes of patients with adenocarcinoma of the cecum. MATERIALS AND METHODS: This retrospective study was carried out at three large tertiary university hospitals. We analyzed the characteristics, prognostic factors, and survival of 162 patients with adenocarcinoma of the cecum that were treated and followed up between 2000 and 2013. All the patients had undergone a right hemicolectomy and received chemotherapy with (n = 48) or without (n = 114) adjuvant radiation therapy. RESULTS: The subjects were 65 females and 97 males with a median age of 56 years (range, 17 to 90 years) at diagnosis. The 5-year local control (LC), disease free survival (DFS), and overall survival (OS) rates were 72.7%, 57.2%, and 62.6% respectively. In a multivariate analysis, age, tumor stage, node stage, and adjuvant radiation therapy were determined to be independent prognostic factors. Age more than 55 years (hazard ratio [HR] = 1.0; 95% confidence interval [CI], 0.06–0.32; p = 0.003], T4 stage (HR = 6.8; 95% CI, 3.07–15.36; p < 0.001), node positive disease (HR = 4.2; 95% CI, 1.94–9.13; p < 0.001), and the absence of adjuvant radiation therapy (HR = 3.0; 95% CI, 1.39–6.46; p = 0.005) had a negative influence on OS. CONCLUSION: Adjuvant radiation therapy significantly improves DFS and OS in patients with adenocarcinoma of the cecum.
